style="font-size:10.5pt;font-family:Calibri;text-justify:inter-ideograph;text-align:justify;margin-bottom:0.0001pt;margin:0pt;text-indent:21pt;height:1.5;">（1）申請人應(yīng)為中華人民共和國境內(nèi)（不含香港、澳門、臺灣地區(qū)）法律上和財務(wù)上獨立的法人或依法登記注冊的組織，合法運作并獨立于招募人和招募代理機(jī)構(gòu)。申請人應(yīng)具有良好的銀行資信和商業(yè)信譽(yù)。

style="font-size:10.5pt;font-family:Calibri;text-justify:inter-ideograph;text-align:justify;margin-bottom:0.0001pt;margin:0pt;text-indent:21pt;height:1.5;">申請人為中華人民共和國境內(nèi)（不含港澳臺地區(qū)）法律上和財務(wù)上獨立的法人或依法登記注冊的其他組織，合法運作并獨立于采購人和采購代理機(jī)構(gòu)。（申請人為分公司的，應(yīng)提供總公司和分公司的上述證明文件，及總公司授權(quán)分公司申請本項目的授權(quán)書原件，并加蓋申請人公章）。

style="font-size:10.5pt;font-family:Calibri;text-justify:inter-ideograph;text-align:justify;margin-bottom:0.0001pt;margin:0pt;text-indent:21pt;height:1.5;">①如應(yīng)答人為增值稅一般納稅人，且加載統(tǒng)一社會信用代碼的相關(guān)證照上未加蓋一般納稅人戳印的，要求提供稅務(wù)機(jī)關(guān)官網(wǎng)一般納稅人資格查詢結(jié)果截圖或稅務(wù)機(jī)關(guān)出具的有效證明材料；
style="font-size:10.5pt;font-family:Calibri;text-justify:inter-ideograph;text-align:justify;margin-bottom:0.0001pt;margin:0pt;text-indent:21pt;height:1.5;">②如應(yīng)答人為小規(guī)模納稅人，要求提供為本項目使用增值稅專用發(fā)票進(jìn)行結(jié)算的承諾函（格式自擬，需加蓋公司公章）；
style="font-size:10.5pt;font-family:Calibri;text-justify:inter-ideograph;text-align:justify;margin-bottom:0.0001pt;margin:0pt;text-indent:21pt;height:1.5;">（3）申請人須提供公司整體實力及服務(wù)能力的介紹（內(nèi)容須包括貨物或者設(shè)備的供應(yīng)目錄，但不限于公司背景介紹、以往業(yè)績、服務(wù)客戶、資源分布等情況，格式自擬）。
